W 9 Forms for 10-99s
It has been quite a while since we have asked for you to file W 9 form Request for Taxpayer Identification Number. We need to update our files. We are required by the Federal Government to file 10-99s for attorneys and law firms that receive money from our office. Please file or re-file a new W 9 with our office as soon as possible. Thank you! Child Support IWO- Please use the new forms for income withholding orders. The old form can no longer be accepted. New forms can be found at: http://www.in.gov/dcs/3039.htm.

Confidential Information
Filings containing confidential information per LR71-FL00-403.1.6 & Administrative Rule 9 must be submitted for filing on GREEN paper. We are still receiving filings on white paper that are confidential.

Notice of Appeal
As of January 1, 2012, a party wishing to appeal a court decision must file the Notice of Appeal with the Clerk of the Indiana Supreme Court not the trial court clerk. A copy of the Notice must be served on the trial court clerk, court reporter and judge of the trail court. (Appellate Rule 24(A)(1)
